Care Management offered to individuals throughout Oswego County to assist in meeting basic needs.
Get Set! is for anyone with a mental diagnosis needing help with healing individually and at home. We will assist you and your household in improving health, safety, and well-being with skills and tools to cope.
Open to the community and offering clothing, housewares, and more at low prices.
Serving families throughout Oswego County who have an open Preventive or Foster Care Case and children from birth to age 18.
Designed to help parents and caregivers of young children learn how to provide a loving, safe and secure base for their children.
A place for children to come explore in various areas of development through play, activiites and social interactions.
Care Management services for adults with either a mental health diagnosis and/or chronic health condition and medicaid.
Care Management services for children with either a mental health diagnosis and/or chronic health condition and medicaid.
Offering transportation to adults and youth to various appointments. Referrals come from DSS Department of Mental Hygiene
Visit facilitation provided for parents/children and/or siblings. Referrals are received from Department of Social Services.
